IUB-David P. Charlesworth Memorial Scholarship
The David P. Charlesworth Memorial Scholarship is awarded to an undergraduate student in the O’Neill School of Public and Environmental Affairs who has demonstrated financial need and is working at least 20 hours per week to put themselves through school. Documentation of employment must be provided in the form of a check stub or an official letter from the student’s employer.

- Please upload one document that outlines the amount of gross earnings that you, the student, received covering prior year’s employment. Please include official tax forms, such as a W2, for any jobs held during the prior year. Alternatively, you can upload a payroll or the last paycheck you received during the prior year that clearly shows your Year-To-Date earnings. Please be sure to black out your Social Security or banking numbers so that they are not visible when you upload this document. If you completed work for which you did not receive a W2 of paychecks, such as babysitting, working on your family farm, etc., please include a spreadsheet with detailed descriptions of the earnings you received for this type of work.
