IUFW-Patricia Cook Hayduk Dental Alumni Scholarship

Overview:

This scholarship, donated by the Hayduk family, is intended to support students enrolled full-time in the Dental Hygiene Program at the School of Dentistry on the Fort Wayne campus who are Dental Hygiene I students preparing to become Dental Hygiene II students in the next school year.

Eligibility:

You must be:

• Enrolled full-time
• Entering the 2nd year of the Dental Hygiene program
• Have a minimum GPA of 2.7
• Be an Indiana Resident
• Exhibit Leadership, Compassion, and Enthusiasm for Dental Hygiene
• Living on Campus

Awarding:

This scholarship is awarded for the following academic year.

Renewal:

This award is not renewable.
