Letter of recommendation from a faculty member or supervisor familiar with your academic record is required:

You are responsible to check if the reference letter has been submitted. Double check the email address you enter, if an incorrect email is entered, there is no system error. You will be responsible to follow up with your reference and communicate with them about the request for a letter of recommendation and what it should address.

In addition, a statement written by the nominee (no more than two pages, double-spaced, in Times New Roman or Arial 12pt font)

i) The statement should showcase your work in at least one of the areas above with clear explanation of how it goes above and beyond what is expected from your program and laboratory work

ii) Discuss the impact your work has on the department, IUPUI, or the community, if your work showed initiative such as taking on leadership roles, and how it demonstrates your commitment to diversity, equity, and inclusion

iii) If applicable, you are allowed to highlight more than one way in which you feel you have promoted diversity, equity, and inclusion (e.g., engaged in inclusive mentoring and conducted outreach to the community or conducted different types of outreach)
